Immunizations Required For School Registration or Attendance
2008-2009

Kindergarten

4 DTaP if the last dose given after the age of four years
3 TOPV/IPV (Polio) with the last dose after the age of 4 years-if all are given before age 4
2 MMR received at or after one year of age (at least 28 days apart)
3 Hep B- Given at the recommended intervals
2 Varicella (chickenpox) received on or after the 1st birthday and at least ** 28 days apart OR signed verification of disease from the doctor

First and Second Grade

3 DTaP with the last dose after 4 years of age
 3 TOPV/IPV (Polio) with the last dose after the age of 4 years-if all are given before age four
2 MMR received at or after one year of age (at least 28 days apart)
3 Hep B Given at appropriate intervals
1 Varicella (chickenpox) given on or after the 1st Birthday **OR approximate age or date of chickenpox disease is acceptable

Third Through Fifth Grade
3 DTaP with the last dose after 4 years of age
3 TOPV/IPV (Polio) with the last dose after the age of 4 years-if all given before age 4
2 MMR received at or after one year of age (at least 28 days apart)
3 Hep B Given at appropriate intervals

Sixth Grade

3 DTaP with the last dose after the age of 4 years
1 Tdap if it has been 5 years or more since last tetanus containing vaccine and student is 11 years or older
3 OPV/IPV (Polio) with the last dose after the age of 4 years, if all are given before age 4
2 MMR received at or after one year of age (at least 28 days apart)
3 Hep B Given at the recommended intervals
1 Varicella (chickenpox) given on or after the first birthday OR approximate age or date of chickenpox disease is acceptable

Recommended Hepatitis B intervals
The minimum interval between dose #1 and dose #2 is 4 weeks (24 days)
The minimum interval between dose #2 and dose #3 is 8 weeks (52 days)
The minimum interval between dose #1 and dose #3 is 4 months
The minimum age for dose #3 is 24 weeks of age (164 days)
